You may choose to take your child to their primary care physician for their physical and have him/her fill out the Medical History form. The Medical History form can be found on the HISD website under Henderson High School- Athletics- Athletic Forms. Dear Parent/Guardian,
It’s that time of year again! Please remember to pick up your child’s medication(s) from the Nurse’s Office before school gets out for Summer. Medication not picked up will be discarded May 28th.
If your child is a car rider, medication can be sent home in their backpack as long as you send a note stating to do so. If your child rides the bus, you must pick up the medication at your child’s campus. Medication is not allowed on the bus.

Si su hijo/a se va en carro, los medicamentos se pueden enviar a casa en su mochila siempre que envié una nota indicándolo. Si su hijo/a se va en autobús, debe recoger el medicamento en el campus de su hijo/a. No se permite medicamentos en el autobús.
